{"id":23307,"date":"2026-08-21T17:28:35","date_gmt":"2026-08-21T15:28:35","guid":{"rendered":"https:\/\/aminogram.com\/?p=23307"},"modified":"2026-08-21T17:35:44","modified_gmt":"2026-08-21T15:35:44","slug":"bioelectrical-impedance-analysis-clinical-practice-guidelines","status":"publish","type":"post","link":"https:\/\/aminogram.com\/en\/bioelectrical-impedance-analysis-clinical-practice-guidelines\/","title":{"rendered":"Bioelectrical Impedance Analysis: A tool recommended by national and international clinical practice guidelines"},"content":{"rendered":"<p>Over the past two decades, bioelectrical impedance analysis (BIA) has become a mature technology, supported by abundant scientific literature and integrated into numerous clinical practice guidelines.<br \/>\nBy standardizing its modalities of use, these recommendations strengthen its scientific legitimacy, harmonize clinical practices, and promote its adoption in the assessment of body composition.<\/p>\n<section aria-labelledby=\"premieres-reconnaissances\">\n<h2 id=\"premieres-reconnaissances\">The first recognitions of BIA in clinical practice guidelines<\/h2>\n<p>Between 2002 and 2004, ESPEN (European Society for Clinical Nutrition and Metabolism), the leading European scientific society in clinical nutrition, brought together an international working group composed of BIA experts, including several authors of reference equations and methods, notably Kyle, Deurenberg, Elia, Scharfetter, and Pichard. The objective was to critically evaluate the available scientific data in order to establish the physical foundations of the method, to specify its performance, clinical indications, limitations, and the conditions necessary for its standardized use. This work led to the publication, in 2004 in Clinical Nutrition, of two complementary articles: Bioelectrical impedance analysis \u2013 Part I: Review of Principles and Methods<sup class=\"citation\"><a href=\"#ref1\" aria-label=\"Reference 1\">1<\/a><\/sup> and Part II: Utilization in Clinical Practice<sup class=\"citation\"><a href=\"#ref2\" aria-label=\"Reference 2\">2<\/a><\/sup>. These two publications remain today foundational references for the standardization of BIA.<\/p>\n<\/section>\n<section aria-labelledby=\"referentiels-actuels\">\n<h2 id=\"referentiels-actuels\">Current guidelines recommending the use of BIA in clinical practice<\/h2>\n<p>BIA is mainly integrated into recommendations and diagnostic algorithms in the context of malnutrition and obesity.<\/p>\n<p>The field of clinical nutrition has gradually structured around several international reference scientific societies, including ESPEN, ASPEN (American Society for Parenteral and Enteral Nutrition), FELANPE (Federaci\u00f3n Latinoamericana de Terapia Nutricional, Nutrici\u00f3n Cl\u00ednica y Metabolismo), and PENSA (Parenteral and Enteral Nutrition Society of Asia). In a collaborative international effort among these four major scientific societies, the Global Leadership Initiative on Malnutrition (GLIM) was created in 2016 to harmonize the diagnosis of malnutrition in adults on an international scale.<\/p>\n<p>The GLIM criteria<sup class=\"citation\"><a href=\"#ref3\" aria-label=\"Reference 3\">3<\/a><\/sup>, first published in 2019, standardized the diagnosis of malnutrition. BIA was recognized as a method for assessing muscle mass, notably through two indicators, the Fat-Free Mass Index (FFMI) and the Appendicular Skeletal Muscle Index (ASMI). Subsequent publications in 2022<sup class=\"citation\"><a href=\"#ref4\" aria-label=\"Reference 4\">4<\/a><\/sup> and 2025<sup class=\"citation\"><a href=\"#ref5\" aria-label=\"Reference 5\">5<\/a><\/sup> have maintained BIA among the usable methods for evaluating the muscle mass criterion. These recommendations, published in two reference journals in clinical nutrition, Clinical Nutrition and JPEN (Journal of Parenteral and Enteral Nutrition), demonstrate the recognition of this method by the main international clinical nutrition communities.<\/p>\n<p>In addition to these international initiatives, ESPEN has published several clinical practice guidelines over the years supporting the use of BIA. In 2015, in the context of diagnosing malnutrition, ESPEN specifically mentioned the FFMI<sup class=\"citation\"><a href=\"#ref6\" aria-label=\"Reference 6\">6<\/a><\/sup>. In 2017, in its publication dedicated to definitions and terminology in clinical nutrition<sup class=\"citation\"><a href=\"#ref7\" aria-label=\"Reference 7\">7<\/a><\/sup>, BIA was recommended for the assessment of malnutrition, sarcopenia, and sarcopenic obesity, as well as for monitoring patients during their nutritional management and treatment. These recommendations were subject to a high consensus, with 97% agreement. That same year, for cancer patients, ESPEN formulated specific recommendations in favor of an objective and quantitative assessment of muscle mass<sup class=\"citation\"><a href=\"#ref8\" aria-label=\"Reference 8\">8<\/a><\/sup>.<\/p>\n<p>In 2022, ESPEN and the European Association for the Study of Obesity (EASO) established a consensus on the diagnosis of sarcopenic obesity (SO), defined by the coexistence of excess adiposity and decreased muscle mass and function. The diagnosis notably integrates fat mass (%FM) and skeletal muscle mass adjusted for weight (SMM\/W), which can be evaluated by BIA<sup class=\"citation\"><a href=\"#ref9\" aria-label=\"Reference 9\">9<\/a><\/sup>.<\/p>\n<p>ASPEN&#8217;s position regarding BIA has gradually shifted towards recognizing its value in clinical practice. In 2019, a systematic review<sup class=\"citation\"><a href=\"#ref10\" aria-label=\"Reference 10\">10<\/a><\/sup> did not yet allow for the recommendation of its systematic use, mainly due to the heterogeneity of devices and proprietary equations, as well as their limited validation in different populations. Since then, the evolution of available data has led to a more favorable place for BIA in international guidelines. In the GLIM guidelines published in 2022<sup class=\"citation\"><a href=\"#ref4\" aria-label=\"Reference 4\">4<\/a><\/sup>, BIA is explicitly recognized as a method to assess or estimate skeletal muscle mass. This evolution highlights the growing integration of BIA in the assessment of muscle mass and malnutrition, while maintaining the need for standardization of methods and interpretation adapted to the clinical context.<\/p>\n<p>Nutritional assessment by BIA has also been studied in various pathologies. In 2014, the ERS (European Respiratory Society) recommended its use, notably through FFMI, SMMI (Skeletal Muscle Mass Index), ASMI, and fat mass (FM), for the nutritional and prognostic assessment of patients with chronic obstructive pulmonary disease (COPD)<sup class=\"citation\"><a href=\"#ref11\" aria-label=\"Reference 11\">11<\/a><\/sup>. Similarly, the AND (Academy of Nutrition and Dietetics) guidelines concerning cystic fibrosis<sup class=\"citation\"><a href=\"#ref12\" aria-label=\"Reference 12\">12<\/a><\/sup> and HIV infection<sup class=\"citation\"><a href=\"#ref13\" aria-label=\"Reference 13\">13<\/a><\/sup> include BIA among the nutritional assessment methods.<\/p>\n<p>In Asia, there is a growing interest in the use of BIA in critically ill patients, as evidenced by a Chinese clinical practice guideline on nutritional assessment and monitoring<sup class=\"citation\"><a href=\"#ref14\" aria-label=\"Reference 14\">14<\/a><\/sup>, which recommends the use of BIA for nutritional risk screening and skeletal muscle mass monitoring, as well as a Korean single-center observational study<sup class=\"citation\"><a href=\"#ref15\" aria-label=\"Reference 15\">15<\/a><\/sup> on the utility of phase angle and the ECW\/TBW ratio for nutritional assessment and prognosis prediction in intensive care patients.<\/p>\n<\/section>\n<section aria-labelledby=\"recommandations-experts\">\n<h2 id=\"recommandations-experts\">Expert recommendations for BIA applications in research and clinical practice<\/h2>\n<p>The series of publications initiated in 2025 by Prado, Heymsfield, Gonzalez, and Norman<sup class=\"citation\"><a href=\"#ref16\" aria-label=\"Reference 16\">16<\/a>,<a href=\"#ref17\" aria-label=\"Reference 17\">17<\/a><\/sup>, bringing together nearly 30 international experts in body composition, highlights the complementarity between expert groups and scientific societies. It aims to address a recurring problem: the terminological and methodological confusion surrounding body composition assessment. It returns to the methodological foundations of the discipline by defining the different levels of body organization, harmonizing terminology, and specifying, for each measurement method\u2014bioimpedance, DXA, CT, and ultrasound\u2014its principles, validity, reliability, and limitations. This series also reflects the evolutions in the field over the past twenty years since the 2004 ESPEN guidelines<sup class=\"citation\"><a href=\"#ref1\" aria-label=\"Reference 1\">1<\/a>,<a href=\"#ref2\" aria-label=\"Reference 2\">2<\/a><\/sup>, notably by clearly distinguishing BIA (SF\/MF) from BIS and addressing the limitations related to devices whose proprietary algorithms for data processing and interpretation are not fully transparent.<\/p>\n<\/section>\n<section aria-labelledby=\"place-francaises\">\n<h2 id=\"place-francaises\">The place of BIA in French guidelines<\/h2>\n<p>BIA is part of current French guidelines, both for the diagnosis of malnutrition and the assessment of sarcopenic obesity. The HAS (French National Authority for Health), in collaboration with the French Federation of Nutrition (FFN), has published several good practice guidelines integrating the use of BIA: for malnutrition in people aged 70 and over in 2021<sup class=\"citation\"><a href=\"#ref18\" aria-label=\"Reference 18\">18<\/a><\/sup>, in adults under 70 in 2019<sup class=\"citation\"><a href=\"#ref19\" aria-label=\"Reference 19\">19<\/a><\/sup>, and in adult obesity in 2022<sup class=\"citation\"><a href=\"#ref20\" aria-label=\"Reference 20\">20<\/a><\/sup>. Parameters such as FM%, SMM\/W, FFMI, and ASMI are included in the HAS guidelines.<\/p>\n<\/section>\n<section aria-labelledby=\"en-resume\">\n<h2 id=\"en-resume\">In summary<\/h2>\n<p>The integration of BIA into the recommendations of scientific societies and reference organizations represents a major step forward for this technology.
